What is the Eastern Counties Hospice Palliative and End-of-Life Care Network?

1. Purpose
 
A local systems planning structure for the Eastern Counties is an integral component of the overall end-of-life service delivery model that will ensure the Ministry of Health and Long-Term Care principles of health system transformation will continue to be reflected in the implementation and evaluation of the model over time. These principles include:

  • Equitable access based on patient needs
  • Preserves patients’ choice
  • Measurable results-driven outcomes based on strategic policy formulation, business planning and information management
  • People-centred, community-focused care that responds to local population health needs
  • Shared accountability between providers, government, community and citizens


2. Vision

Our Eastern Counties Hospice Palliative and End-of-Life Care Network will establish a high quality, accessible, and coordinated continuum of hospice palliative and end-of-life care for every person of every age.

3. Network Structure and Functions Operationalizing the “ Square Of Organization ” Locally

The Eastern Counties Hospice Palliative and End-of-Life Network is a community-based vehicle that facilitates people, ideas and resources coming together to improve palliative care services for clients and their families in the Eastern Counties. It is a forum for collaboration amongst providers of palliative care in the Eastern Counties. Established in September 2005, the key functions are to:
  • Identify issues/gaps and local priorities;
  • Provide feedback and input into planning;
  • Create education and training opportunities; and
  • Serve as the link to the local community from the Champlain Hospice Palliative and End-of-Life Care Network with respect to district wide initiatives and direction.
Specific activities:
  • Provide strategic guidance and direction for the implementation of the Integrated Service Delivery Model.
  • Maintain the Eastern Counties Hospice Palliative and End-of-Life Care Website (www.epalliativecare.ca)
  • Provide representation on, and work in concert with, the Champlain Hospice Palliative and End-of-Life Care Network and its committees/work groups.
  • Oversee the use of funds to support identified activities, projects and initiatives as they relate to education and training under Initiatives #1 and #2. 

Membership:

Membership takes into account geography, health sector and service perspective.

It has broad and open membership across the Eastern Counties to allow for participation by all those who have an interest in end-of-life palliative care. It includes palliative care providers in the Eastern Counties and has representation from hospitals, physicians, CCAC, LTC Homes and Community Support Services, as well as community representatives (informal caregivers/volunteers) from across the five Eastern Counties. A nomination process for community representatives will be used.
