This website is a joint project of the Eastern Counties Hospice Palliative and End-of-Life Care Network and the Champlain Community Care Access Centre (CCAC).

Its PRIMARY goal is to provide information to health care professionals, volunteers, caregivers and the public about palliative care and local community resources. In addition, it serves as a forum for advising of educational opportunities and new initiatives, as well as for coordinating the work of the Network.
